District & Volunteer Vehicle Accidents & Insurance
—Reporting an Accident

  1. All accidents involving 4J vehicles, including rental vehicles, require the driver (employees & volunteers) to report the accident immediately to his/her supervisor and to the 4J Transportation office at 687-3435. Failure to report an accident is a serious violation of the District 4J Vehicle Accident policy.

  2. Fill out the Vehicle Accident Information Form (to assist you in exchanging driver information) located in the glove compartment of all District vehicles commonly used to transport students. Accident reports can be obtained at the Transportation Department main office located at 1938 W. 8th. The employee should call (687-3425) for an appointment. The accident report should be filled out by the employee at Transportation within a 24 hour period after the accident and submitted to the Training Coordinator or Transportation Supervisor.
