The 4J Wellness Clinic–The clinic provides a full range of primary health care, diagnostic tests, minor surgery and preventative care to insurance-eligible 4J employees, retirees and their families with pre-paid medical care for routine needs. Annual physicals are available, as well as school, sports and camp physicals for children.

Tobacco Cessation - OPTIONS Program(pdf) - This is a community program offered by the Oregon Heart & Vascular Institute in affiliation with Sacred Heart Medical Center. This program consists of:

• Individual Counseling
• Support for Self Help
• Behavioral Therapy Sessions including four 75-minute sessions once a week

The OPTIONS program can be started at any time and is FREE TO ANY 4J EMPLOYEE regardless of benefits eligibility. Simply identify yourself as a 4J employee and the $45 enrollment fee will be billed to the district.  (Dependents of employees may participate, but are not eligible for the fee waiver.)
